The following items are required for ALL incoming interns. There are specific instructions printed on each form. Please carefully read each form before completing and always keep copies of everything you fax or mail to our office. If you are under the age of 18, both parents/guardians signatures are required.
All paperwork is due by Friday, December 7, 2012.
1. Financial Agreement
The Financial Agreement contains our financial policies and the Weighted Donation Schedule. This form is required prior to submitting your Housing Deposit and/or participation in any scholarship program.

2. Medical Assessment
Please complete the Medical Assessment form. You will need to schedule a doctor’s appointment and receive a complete physical examination before Arrival Day.
Immunization Requirements
The Honor Academy strives to maintain the best possible health for all of our interns. This includes proper immunizations PRIOR to attending the Honor Academy. The following list ARE REQUIRED for all incoming interns. These are required based on recommendations from the American Academy of Pediatrics, the American College Health Association, the Texas Department of State Health Services, and the Center for Disease Control and Prevention.
- -Tetanus-Diphtheria (Td) booster*
- -Meningococcal vaccine
- -Hepatitis B series
- -Measles-Mumps-Rubella (MMR) vaccine series
- -Polio (IPV/OPV) series
Exclusions from compliance are allowable on an individual basis for medical contraindications, reasons of conscience, including a religious belief, and active duty with the Armed Forces of the United States. Please complete the Vaccine Exemption Affidavit form (see below) to satisfy the immunization requirements.
3. Proof of Graduation
All incoming interns must provide evidence of high school graduation or equivalent. We accept one of the following to serve as proof of graduation:
- An original or copy of an official, final, high school transcript.
- A copy of a ‘Certificate of General Educational Development’ or similarly titled credential indicating that the incoming intern has met or exceeded passing score requirements for the GED test.

The Honor Academy recognizes that transcripts from homeschool students may vary widely from one family to another. However, all home school transcripts must meet certain minimum standards to facilitate the enrollment process.
Please submit homeschool records with the following guidelines:
- Homeschooling records should indicate the courses completed; the credits, units, or hours associated with each course; and a letter or numerical grade indicating the student’s level of performance in that subject area for that year.
- Courses or subjects reflected on the transcript should be organized by grade level (9 through 12) and date.
- To be considered “final”, transcripts must include a “Graduation Date” and a signature from the school administrator (usually a parent).

4. Proof of Health Insurance
The HA requires that all interns carry active health insurance for the duration of their internship.
We accept one of the following to serve as proof of health insurance:
- A copy of the intern’s health insurance card (front and back).
- A letter from the insurance company indicating active coverage including key policy details including provider’s name, provider’s phone number, policy number, and policy holder’s name.

In most cases, interns will be covered under their parent/guardian’s insurance policy. However, some insurance companies discontinue coverage when an individual turns 18 and is not a full-time student.

Insurance Options
In most cases, interns will be covered under their parent/guardian’s insurance policy. However, some insurance companies discontinue coverage when an individual turns 18 and is not a full-time student. The Honor Academy currently provides two options to incoming interns who may lose or do not currently have health insurance coverage.
Option #1: Verify Enrollment & Continue Coverage
The Honor Academy has found that most insurance companies will continue coverage if they receive a letter verifying active enrollment in the HA.
To request this letter please complete the online Insurance Letter Request form.
Option #2: Purchase Short-term Health Insurance
In the event the insurance company does not accept your request to continue coverage, or you do not have insurance, you will need to purchase a short-term health insurance plan.The companies below understand the Honor Academy program and are able to assist you in selecting a plan that fits your budget and needs. The list below are recommendations only. The HA does not require a minimum level of coverage.

5. General Release
The General Release form is a required form for all incoming interns. Please note that you will need to sign this form in the presence of a notary and submit the original notarized copy. Please review completion and submission instructions located on the form.
We will be providing a notary on arrival day for incoming interns unable to get this document notarized by the Paperwork deadline. Please note that this will add 30 – 90 minutes to the registration process.
